Mini-Grants
The Indiana Association for Health, Physical Education, Recreation, and Dance (IAHPERD) invites proposals to request funding for program or curriculum development, public information, workshops, or research projects which focus upon wellness, movement, and the development of knowledge, attitudes, and skills necessary to promote and improve overall health and quality of life. Collaborative projects between schools (public and private) and colleges/universities or between schools and related organizations are highly desired.

Eligibility
Proposals will only be accepted from IAHPERD members. Representatives of organizations providing health, physical education, recreation, dance, or related services are eligible to apply for funding. IAHPERD membership information can be obtained from Karen Hatch, Executive Director, 2007 Wilno Drive, Marion, IN 46952. Higher priority will be given to grant proposals that incorporate matching contributions from a supporting organization (i.e., Parent-Teacher Association, School Corporation, community business). Applicants must be willing to present their project at the IAHPERD state conference in November or publish their results in the Indiana AHPERD Journal. An individual (the principal applicant) may not receive funding for more than two grants in a five-year period.
Application Proposal
Applications should include all information as described. Five copies of the completed application with project details and a projected budget must be sent to the Mini-Grant Committee Chair prior to February 1, April 1, August 1, and December 1. Complete application proposals should include:
I. Application Cover Sheet (Form A) – (form is below) Includes All forms (A,B,C, and attachements) mentioned below also (please send together)
II. Project Details (Attach pages explaining the following information)
A. Rationale and Objectives
Provide the rationale for why the project is particularly important. Identify precisely the need of the project and the population to be served. Also indicate the specific objectives of the project.
B. Procedures/Methods
Explain precisely and comprehensively the procedures/methods of the project, i.e., who will be the participants, how will activities be administered, what is the design of the project, etc. Provide a timeline for administering the project.
C. Evaluation
Explain the tools/instruments to be used to measure the project objectives. When and how often will evaluation be administered?
III. Budget (Form B)
In addition to Form B, provide a Budget Summary describing each line item of the budget highlighting the total costs related to the project regardless of whether the request for IAHPERD funding is for total or partial support. Depending on funds available, grant proposals can request up to $2000. All budget amounts should be reported in whole dollars. Applicants are encouraged to secure matching contributions from a supporting organization (i.e., Parent-Teacher Association, School Corporation, community business). The budget form is divided into two sections: Personnel and Operation. The Personnel section contains information regarding personnel needs. Funds may support the work of a project director, secretary, computer programmer, and/or presenter. The Operation section contains information regarding operating expenses, i.e., educational and/or evaluation materials, communication, facility rental, supplies, equipment, travel, etc. Mileage should be based on a maximum allowance by state agencies in Indiana (28 cents per mile), food ($30 per day), and/or lodging ($79 per night). The individual must be over 50 miles from his/her place of residence to budget for overnight lodging. Monies may not be used for salaries and fringe benefits, entertainment, routine travel, expenses for presentation of results, or for replacement or reduction of costs already covered through other funds. Grant funds requested and matching contributions from supporting organizations must be listed and a total cost for Personnel and Operation must be shown.
IV. Supporting Materials
This section should include relevant documents listed in the Project Details and other endorsement materials (i.e., evaluation instruments, job descriptions, letters of support).
V. Conditions of Agreement Covering Mini-Grant Award (Form C)
The applicant should sign and attach this form indicating his/her agreement with the conditions of the grant. If the proposal is funded, the following reports will be required:
A Financial/Progress Report must be submitted to the Executive Director six (6) months after the grant period begins (see Attachment I).
A Final Financial Report is to be submitted to the Executive Director no later than thirty (30) days after the grant period ends (see Attachment II).
A Final Project Results is to be attached to the Final Financial Report. This should provide a significant summary or supporting data of the project.
A written request for an extension of the grant period must be submitted to the Executive Director thirty (30) days prior to the end of the grant period to be considered.
To finalize the project, the applicant will present the results in an Indiana AHPERD Journalarticle or give a brief oral report on the project at the annual IAHPERD state conference in November. The Mini-Grant Chair will be responsible for planning and coordinating this conference session. This session will include all grant recipients from the proceeding year.
Evaluation Procedures
The Mini-Grant Committee will consider all submitted proposals, judge their merit, and rank the selected proposals after each submission date. All proposals will be judged for completeness on the criteria listed on the attached evaluation form. It is important that the applicant be thoroughly familiar with the Mini-Grant Application Guidelines, as the evaluation criteria go hand-in-hand with these requirements.
The Chair of the Mini-Grant Committee may make contact, if circumstances suggest that a follow-up to the applicant is needed after receiving the proposal. Several outcomes are possible from the review process:
Request for additional information.
A request to modify the proposal, usually in terms of limiting project activities or decreasing the amount of funds requested.
Notification of the proposal’s rejection.
The Executive Director will receive ranked recommendations from the Mini-Grant Committee within fifteen (15) days of each submission date. The Executive Director will then submit proposals to the Board of Directors and/or the Executive Committee for consideration at their next scheduled meeting. The Executive Committee will consider requests for $500 or less for funding approval. Requests for over $500 requires approval by both the Executive Committee and the Board of Directors. Upon approval, the Executive Director will direct the dispersal of funds. Proposals will be funded no later than thirty (30) days from date of approval. The grant period begins when the Project Director receives notification of funding from the Executive Director.
Dispersal of Funds
Once the Executive Committee and/or Board of Directors approve an application, it will become the basis for a signed agreement between IAHPERD and the applicant.
When notified that a grant has been approved, the applicant will write to the Executive Director to request payment. Grant funds will be dispersed to the sponsoring organization. Personal checks are NOT written. The transfer of funds from one major section to another requires submission of a revised budget with rationale for the transfer of funds and approval by the Executive Director. Funds not expended by the end of the grant period will be terminated thirty (30) days after the grant period ends unless the Executive Director approves an extension.
